What Does This Privacy Notice Cover?

US Big Deals Inc. (“US Big Deals” or we” or “us”) respects your privacy and are committed to protecting it through our compliance with this privacy notice. US Big Deals is not a veterinarian or provider of other pet services. US Big Deals provides its customers with the ability to obtain veterinarian or other pet services (collectively, the “Pet Services”) by independent practitioners, including, but not limited to, services provided by Community Veterinary Clinics LLC (“PetIQ”). The term “Pet Services” also include information we may collect when you make an appointment online to request Services. US Big Deals does, however, control the means and purposes of PetIQ’s and Walmart’s processing of the personal information collected about you (and your pet) when you use the Pet Services and is responsible for providing you with this Privacy Notice. By using the Pet Services, you agree and acknowledge that PetIQ or other independent practitioners may provide us with Personal Information related to your use of the Pet Services and that we may use such Personal Information as described in this Privacy Notice.

This Privacy Notice (our “Privacy Notice”) describes the types of information we may collect, use, maintain, protect, disclose, or otherwise process about you when you use the Pet Services and our practices for collecting, using, maintaining, protecting, disclosing, or otherwise processing that information.

This policy applies to information we collect:

  • on or through your use of the Pet Services;
  • in email, text, and other electronic messages between you and US Big Deals or PetIQ related to your use of the Pet Services through any medium;
  • when you interact with our advertising related to the Pet Services on third-party websites, if those applications or advertising include links to this policy; and
  • any other information we may collect about you offline when you use the Pet Services.

It does not apply to information collected by:

  • us through any other means, including on any other website or other products and services we offer unrelated to the Pet Services operated by US Big Deals or PetIQ or any third-party (including our affiliates and subsidiaries). For example, this Privacy Notice does not apply to any Personal Information we may collect and use related to your other purchases in our stores, or when you use other services such as eyewear or clinical services. How Does US Big Deals Protect the Privacy of Children Related to the Services?

Our Pet Services are not intended to be used by children under 13 years of age. No one under age 13 may provide any information to us regarding the use of the Pet Services. We do not knowingly collect Personal Information from children under 13. If you are under 13, do not register for or otherwise provide any information to use the Pet Services, including your name, address, telephone number, email address, or any screen name or user name you may use. If we learn we have collected or received Personal Information from a child under 13 without verification of parental consent, we will delete that information. What Personal Information Do We Collect and How Do We Collect It?

Throughout this Privacy Notice, the term “Personal Information” means any information that identifies, relates to, describes, references, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular individual or device. However, Personal Information does not include any deidentified or aggregated information.

Generally

We collect Personal Information from various sources, including:

  • directly from you when you provide it to us;
  • information that we create about you as you use the Pet Services; and
  • From third parties, for example, other veterinarians or providers of other pet services.

Information You Provide to Us

We collect the following types of Personal Information directly from you when you register for or use Pet Services: real name, postal address, email address, telephone number, emergency contact information, pet name, pet type (dog, cat, fish), pet breed, pet gender, pet birthday, pet veterinarian, current pet health and medical history. In addition, we also collect other types of information that you may provide when you fill out a form or through your correspondence with us.

We collect Personal Information from you when you use our Pet Services through:

  • Forms in our facilities or online when you register or book an appointment to receive the Pet Services. We may also ask you for information when you report a problem with the Pet Services;
  • if you contact us, records and copies of your correspondence (including email addresses);
  • your responses to surveys that we might ask you to complete for research purposes; and
  • details of the services you receive when you use our Pet Services and fulfillment of your requests (such as medications for your pet, vaccinations, and other procedures).

Information We Create About You

We may also create certain information about you. When we associate this information with other Personal Information about you, we consider this information to be Personal Information. This information includes: records of services obtained or other purchasing or consuming histories or tendencies related to the Pet Services and genetic characteristics, physiological characteristics, behavioral characteristics, biological characteristics, predispositions, and behavior of your pet.

Information We Collect from Third Parties

We may collect Personal Information from third parties. When we associate this information with other Personal Information about you, we consider this information to be Personal Information. This information includes: medical information and history, age, birthdate, color, breed, and gender of your pet.

How Do We Use Personal Information?

We use information that we collect about you or that you provide to us, including any Personal Information:

  • to provide you with and personalize the Pet Services that you request from us;
  • to support, develop, troubleshoot, and debug the Pet Services;
  • to create, maintain, customize, and secure your Pet Services account with us;
  • to provide you with notices about your account, such as recommended treatments and upcoming vaccinations that are due;
  • to provide you with support and to respond to your inquiries, including to investigate and address your concerns and monitor and improve our responses;
  • to help maintain the safety, security, and integrity of the Pet Services and our business;
  • for internal testing, research, analysis, and product development, including to develop and improve the Pet Services;
  • detecting incidents, protecting against malicious, deceptive, fraudulent, or illegal activity, and prosecuting those responsible for that activity;
  • to carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collection;
  • to notify you about changes to the Pet Services;
  • in any other way we may describe when you provide the information;
  • to fulfill any other purpose for which you provide it; and
  • for any other purpose with your consent.

We may also use your Personal Information to contact you about other goods and services related to the Pet Services that may be of interest to you. Who Do We Disclose Personal Information To and Why?

We do not share, sell, or otherwise disclose your Personal Information for purposes other than those outlined in this Privacy Notice. However, we may disclose aggregated information about our users, and information that does not identify any individual, without restriction.

We may disclose Personal Information that we collect or you provide as described in this Privacy Notice:

  • to our subsidiaries and affiliates. Such use by our subsidiaries and affiliates are governed by their applicable privacy notice, which may be found at: .
  • to contractors, service providers, and other third parties we use to support the Pet Services and who are bound by contractual obligations to keep Personal Information confidential and use it only for the purposes for which we disclose it to them. These entities provide veterinary and other pet services or related services, such as shipping and receiving, lab testing, and other similar services;
  • to your regular veterinary or other veterinary or pet services upon your request;
  • to a potential or actual buyer or other successor in the event of a planned or actual merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of Walmart’s or PetIQ’s assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which Personal Information held by US Big Deals or PetIQ about the Pet Services’ users is among the assets transferred;
  • to fulfill the purpose for which you provide it. For example, if you give us the contact information about your current or previous veterinary, we will use your Personal Information to obtain your pet’s previous medical history or otherwise discuss your use of the Pet Services with them;
  • for any other purpose disclosed by us when you provide the information; and
  • with your consent.

We may also disclose your Personal Information:

  • to comply with any court order, law, or legal process, including to respond to any government or regulatory request;
  • to enforce or apply Pet Services terms of use and other agreements, including for billing and collection purposes; and
  • if we believe disclosure is necessary or appropriate to protect the rights, property, or safety of US Big Deals, our customers, your pet, or others. This includes exchanging information with other companies and organizations, such as a local animal welfare agency, for the purposes of protecting your pet from you.

What Choices Do You Have About How We Use and Disclose Your Information?

We do not control the collection and use of your information collected by third parties described above in Disclosure of Your Information. When possible, these organizations are under contractual obligations to use this data only for providing the services to us and to maintain this information strictly confidential. These third parties may, however, aggregate the information they collect with information from their other customers for their own purposes.

In addition, we strive to provide you with choices regarding the Personal Information we have collected about you. We have created mechanisms to provide you with control over your Personal Information:

  • Promotional Offers from Walmart. If you do not want us to use your contact information to promote other related products and services, you can opt-out by contacting us at the contact information below or, if we have sent you a promotional email, clicking the unsubscribe link in the body of that email. This opt out does not apply to information provided to US Big Deals as a result of your use of the Pet Services or your experience when using the Pet Services.

How Can You Access, Delete, and Update Your Personal Information?

You may request access to, or request that we correct or delete any Personal Information that we have collected about you by contacting us at the contact information below. We cannot delete your Personal Information except by also deleting your account. We may not accommodate a request to change or delete your Personal Information if we believe the change or deletion would violate any law or legal requirement or cause the information to be incorrect.

How Do We Secure Your Personal Information?

US Big Deals and PetIQ have implemented measures designed to secure your Personal Information from accidental loss and from unauthorized access, use, alteration, and disclosure.

Although we do our best to protect your Personal Information, we cannot guarantee the security of your Personal Information that we collect when you use our Pet Services. Any provision of Personal Information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures related to the Pet Services.  

How Will You Know if This Privacy Notice Changes?

We may change this Privacy Notice at any time. It is our policy to post any changes we make to our Privacy Notice on this page with a notice that the Privacy Notice has been updated at the point of the Pet Services (in the store, or online if you make an appointment for the Pet Services online). If we make material changes to how we treat our users’ Personal Information, we will notify you by email to the email address specified in your account and/or through a notice where you use the Pet Services (in the store or online if you make an appointment for the Pet Services online). The date this Privacy Notice was last revised is identified at the top of the page. You are responsible for ensuring we have an up-to-date active and deliverable email address for you, and for periodically visiting this Privacy Notice to check for any changes.

Your continued use of the Pet Services following the posting of changes constitutes your acceptance to such changes.
